We work with SOURCE, Home Health Services, Private Home Care and Hospice Agencies, Nursing Homes, Retirement and Assisted Living Communities as well as Personal Care Homes.
We do blood work in your home and write prescriptions.

Dr. Angella Samuels
Dr. Angella Samuels received her doctorate at The University of Alabama— Tuscaloosa
She is a dedicated family nurse practitioner with more than 10 years of clinical experience and her extensive background in family medicine makes her knowledgeable in all aspects of overall health management. She is a member of AANP, UAPRN, and the American Academy of Home Care Medicine.
April Criddell
| April Criddell (Office Manager) has been caring for Alzheimer and Dementia patients since 2012 and with Faith Healthcare Services since 2016. She is a certified EHR specialist and a certified medical administrative assistant with over 20 yrs experience in office administration. She enjoys helping patients and their families. |
Kurlene Nelson Morris
| Kurlene Nelson- Morris (Phlebotomist/ Nursing Assistant) has been at Faith Healthcare since 2011. She is a certified phlebotomist and enjoys working in the medical field and loves caring for all patients especially those that cannot get out of their homes to see a doctor. |
