Once insurance has been verified, all new patients must complete the patient data sheet, comprehensive History and Physical form, Authorization for Release of medical records, as well as Authorization for treatment, and provide a photo ID. All medical records will be obtained before you can be seen by FHS providers or staff. Please be sure to include your email address.
